All Incident Reports must be completed and submitted within 5 Days of when the incident took place.
Please choose from one of the following reports to submit:
- Dog Safety & Control Incident Report
- Injury/Damage Incident Report
- Report of Suggestion/Concern/Complaint
**All reports are forwarded to the applicable Training Committee Director.
Follow up by the Training Committee and/or the BOD will occur when a conclusion is made regarding this incident.
